Bottom Line: Missouri Medicare Savings Programs can help pay Medicare costs for people with limited income and limited resources. The main help is payment of the Part B premium. If you qualify for Qualified Medicare Beneficiary (QMB), Missouri can also protect you from most Medicare deductibles, coinsurance, and copays for Medicare-covered care. This is handled through MO HealthNet and the Family Support Division, not through a local cash grant.
Use Missouri’s current 2026 numbers, not old flyers. The state limit chart lists the current QMB, SLMB, QI, and QDWI limits, and those figures are used throughout this guide.
Emergency help now
- If you have QMB and get a bill: Do not pay first. Call the provider’s billing office and say you are in QMB. Medicare providers must follow QMB billing rules for Medicare-covered services.
- If a collection notice arrives: Call 1-800-633-4227 and say, “I am in QMB and I am being billed for Medicare cost sharing.” Medicare’s QMB billing tips explain the refund and stop-billing steps.
- If you need help applying: Call Missouri SHIP at 1-800-390-3330 for Medicare help, or call the Senior Resource Line at 1-800-235-5503 to reach your local Area Agency on Aging.
- If your case is stuck: Call the Family Support Division at 1-855-373-4636. Missouri’s Resource Center map also lists office and document options.

What this help pays for
Medicare Savings Programs help people with Medicare who have low income and limited resources. In Missouri, the programs are part of the MO HealthNet system for seniors and people with disabilities. You do not get a separate “senior grant” check. The state helps pay certain Medicare costs.
The biggest savings for many people is the Part B premium. Missouri’s 2026 chart lists the standard Medicare Part B premium at $202.90 per month. If MSP pays that premium, more money may stay in your Social Security check or stop being billed to you by Medicare.
QMB is the strongest level. It may pay the Part A premium if you owe one, the Part B premium, and Medicare deductibles, coinsurance, and copays for Medicare-covered care. SLMB and QI pay the Part B premium only. QDWI is different. It pays the Part A premium only for certain working people with disabilities who lost premium-free Part A.
Missouri also uses names that can confuse people. QI may appear as SLMB2 or QI-1 on state papers. SLMB may appear as SLMB1. If you see those names, do not assume your case is wrong. Check the notice and call if the benefit does not match what you expected.
For a broader Missouri benefits path, see our Missouri senior benefits guide. This page stays focused on Medicare Savings Programs.
Income and asset limits
The limits below are Missouri’s public limits for April 2026. They can change when federal poverty levels, Medicare amounts, or state rules change. If your income is close, apply anyway. Missouri uses Medicaid counting rules, and some income may be treated differently than you expect.
| Program | Individual monthly income | Married couple monthly income | Individual resources | Married couple resources |
|---|---|---|---|---|
| QMB | $1,330 | $1,804 | $9,950 | $14,910 |
| SLMB or SLMB1 | $1,596 | $2,164 | $9,950 | $14,910 |
| QI, SLMB2, or QI-1 | $1,796 | $2,435 | $9,950 | $14,910 |
| QDWI | $2,660 | $3,607 | $4,000 | $6,000 |
What may count as resources: bank accounts, cash, certificates of deposit, stocks, bonds, some real estate, and some life insurance value may count. The Medicare cost guide says a home, one car, furniture, household goods, burial plot, personal items, and up to $1,500 set aside for burial per person usually do not count.
Married couples: If you are married and live together, expect Missouri to ask about both spouses’ income and resources. This can include accounts owned by one spouse, both spouses, or jointly with someone else.
January Social Security raises: If you were denied right after a Social Security cost-of-living increase, ask FSD how it counted that increase. Missouri has special timing rules for some MSP income updates.
QMB, SLMB, QI, and QDWI compared
| Program | What it pays | Who it may fit | Key Missouri reality check |
|---|---|---|---|
| QMB | Part A premium if owed, Part B premium, and Medicare cost sharing for Medicare-covered services. | People with the lowest income who need the most protection from Medicare bills. | Missouri’s QMB start rule says QMB is not retroactive before the month after the decision. |
| SLMB or SLMB1 | Part B premium only. | People over the QMB line who still need help with the Part B premium. | The SLMB start rule may allow the application month and prior-quarter coverage if all rules are met. |
| QI, SLMB2, or QI-1 | Part B premium only. | People over the SLMB line who do not qualify for another MO HealthNet assistance type. | The Medicare MSP page says QI must be renewed each year and is first-come, first-served. |
| QDWI | Part A premium only. | Some working people with disabilities who lost premium-free Part A after returning to work. | Missouri’s QDWI rule says QDWI does not give a MO HealthNet card or Part B help. |
Plain-English program notes
Ask for QMB first if bills are the problem. QMB is the level that can stop most Medicare cost-sharing bills for Medicare-covered care. It does not make every service free. It does not cover care Medicare does not cover.
Ask about SLMB or QI if the Part B premium is the problem. These programs can help when the premium is the main strain. If approved, it can take time for the premium change to show up in Social Security or Medicare billing.
Ask about QDWI only if the work-and-disability rule fits. QDWI is not a general senior program. It is a narrow program for certain people who lost free Part A after going back to work.
How to apply in Missouri
Choose the right path before you start. Use the MSP-only form if you only want Medicare premium or cost-sharing help. Use the full healthcare application if you also need MO HealthNet, Spend Down, nursing home help, home care, or other Medicaid screening. Missouri’s own MSP form says it is not the same as applying for healthcare.
- MSP only: Fill out the IM-1MSP form online, or print it and send it with proof.
- MSP plus MO HealthNet: Apply through myDSS healthcare, by phone at 1-855-373-9994, or through a local Resource Center.
- Already on MO HealthNet: File a Change Report. Ask Missouri to review MSP eligibility instead of starting a brand-new case unless FSD tells you to do so.
- Need local help: Your local Area Agency on Aging may help you prepare the application. Our Missouri AAA guide can help you find the right local office.
Missouri accepts documents online, by fax, by mail, or through local offices. The state contact page lists the fax number as 573-526-9400 and the mail address as Family Support Division, P.O. Box 2700, Jefferson City, MO 65102.
Phone script for applying
Call FSD at 1-855-373-4636 and say: “I have Medicare and I want to apply for Medicare Savings Programs. I need to know whether I should use the MSP-only form or the full MO HealthNet application. Can you tell me what proof you need and how I should send it?”
Phone script for checking status
Call FSD and say: “I applied for Medicare Savings Programs on [date]. My name is [name], and my date of birth is [date]. Please tell me whether my application was received, what program levels were checked, and what proof is still missing.”
If you have trouble with online forms, our benefits portal guide explains the main Missouri portals in plain language.
What to gather before you apply
Missing proof is one of the most common delays. Missouri may ask for different proof based on your case. The state verification page explains that FSD may ask you to verify information before benefits can be approved.
| Bring or send | Why it matters |
|---|---|
| Medicare card | Shows Part A and Part B status. |
| Social Security, pension, work, or VA income proof | Helps FSD count monthly income. |
| Bank statements from the last 30 days | Helps prove resources. |
| Life insurance, stocks, bonds, CDs, or property papers | Needed if you own these resources. |
| Spouse income and resource proof | Needed for many married applicants. |
| Other insurance cards or premium notices | Helps Missouri review other health coverage. |
| Authorized representative form | Needed if an adult child, caregiver, guardian, or helper will speak for you. |
| Immigration papers | Needed if the applicant is a qualified non-citizen. |
You can also use our document checklist to make a printable list before you call.
What happens after approval
After approval, Missouri should send a notice that names the program level. Read the whole notice. Look for QMB, SLMB1, SLMB2, QI-1, or QDWI. Also check the start date. The start date affects which bills may still be yours.
For QMB: Coverage starts no earlier than the first day of the month after Missouri makes the eligibility decision. This is why applying quickly matters. QMB usually does not fix old bills from before QMB started.
For SLMB and QI: Coverage may start with the application month and may include prior-quarter coverage if all rules were met then. This can matter if your Part B premium was taken out before approval.
For QDWI: If all factors are met, benefits can begin the first day of the application month. QDWI pays only the Part A premium.
If you get QMB, show your Medicare card and your MO HealthNet or QMB proof every time you get care. If you get SLMB or QI, watch your Social Security check or Medicare bill. It can take time for premium payments to update.
If you also have full MO HealthNet, Missouri’s annual renewal rules still matter. Do not ignore renewal mail. QI also must be renewed each year.
What to do if billing, denial, or delay happens
If a provider bills you after QMB approval
Call the billing office and say: “I am in the Qualified Medicare Beneficiary program. Please check my QMB status and remove Medicare deductibles, coinsurance, and copays for Medicare-covered services. If this bill was sent to collections, please recall it and correct the account.”
If the bill keeps coming, call 1-800-633-4227. Ask Medicare to confirm your QMB status and contact the provider. If you already paid, ask whether you can get a refund.
If Missouri denies the case
Do not stop at “denied” if the notice seems wrong. Call FSD and ask exactly what was counted. Ask which program levels were tested. Ask whether QMB, SLMB, QI, and QDWI were all considered if they fit your situation.
Call script for a denial: “I received a denial for Medicare Savings Programs. Please tell me the income amount you counted, the resources you counted, and which MSP levels were reviewed. I also need to know my appeal deadline.”
Missouri’s state rights page says you have hearing rights if the state denies help, reduces benefits, refuses an application, or does not respond on time. The benefit hearings page explains the appeal process.
If you are delayed or overwhelmed
Call script for local help: “I am a Missouri Medicare beneficiary trying to apply for a Medicare Savings Program. I need help with the form, proof, and a denial or delay. Can you connect me with a counselor or local aging office?”
Use this script with Missouri SHIP, the Senior Resource Line, or your local AAA. Seniors with disabilities may also find related local help in our Missouri disability guide.
Backup options if MSP does not solve the problem
- If drug costs are the main problem: QMB, SLMB, and QI usually bring Extra Help with Part D. If MSP does not work, read our Extra Help guide and apply for drug-cost help separately.
- If you need full medical coverage: MSP may not be enough. Our Medicaid guide explains the wider Medicaid path for older adults.
- If you are over the MSP limit: Ask Missouri whether Spend Down or another MO HealthNet category fits your situation.
- If rent, utilities, food, or a shutoff is the crisis: Use our Missouri emergency help guide for faster local starting points.
- If housing is unstable: Our Missouri housing help guide covers rent, senior housing, and local housing contacts.
- If you are a senior veteran: Our Missouri veteran guide can help with veteran-specific contacts.
- If assisted living is the issue: Our Missouri assisted living guide explains payment limits, MO HealthNet options, and gaps to watch.
Estate recovery note: Missouri’s estate recovery rule says recovery does not apply to people who only received QMB and SLMB. If the person also had full MO HealthNet, nursing home help, or other coverage, ask for case-specific help.

Special notes for common situations
Rural seniors: You do not have to rely only on online forms. Missouri allows phone, mail, fax, upload, and in-person options. Call before visiting an office because local hours can change.
Veterans: VA income still needs to be reported. Bring award letters or benefit statements. MSP can still matter if Medicare premiums are cutting into your monthly income.
Immigrant and refugee seniors: Missouri may ask for immigration proof if you are a qualified non-citizen. Ask FSD for language help if English is a barrier.
Family caregivers: If you are helping a parent, ask whether the senior needs an authorized representative form. Do not send Social Security or Medicare numbers by public contact forms unless the agency tells you how to do it safely.
