Bottom line: Rhode Island runs Medicare Savings Programs through Medicaid under the state MPP page. The biggest 2026 rule is simple: as of February 1, 2026, many people who would have been in SLMB are now put into QMB instead. That can give stronger help with Medicare bills.
If you have Medicare and limited income, do not rule yourself out too fast. Rhode Island has higher 2026 income limits than the basic federal numbers for its main tracks. It also still checks resources for QMB and QI. If you need a wider state benefits overview, see our Rhode Island senior benefits guide.
Urgent help now
- If you have QMB and got a bill: Do not pay it before checking it. Call the billing office and say, “I am a Qualified Medicare Beneficiary. Please rebill this as a QMB case.”
- If your help was denied or closed: Read the notice date. The state appeal rights chart says Medicaid appeals generally use 30 days after the notice date plus five days for mailing. Asking within 10 days can matter if you want aid pending while the appeal is reviewed.
- If you cannot use the online portal: Call DHS at 1-855-697-4347, dial 7-1-1 for relay, or use a DHS regional office. Bring your Medicare card, income proof, and recent account balances.
Quick help
- Fast online path: Apply or upload documents through HealthyRhode RI.
- Paper MSP-only path: Use the MPP-1 form.
- Main DHS phone: 1-855-697-4347.
- Free Medicare counseling: Call the Office of Healthy Aging through OHA Medicare help at 1-888-884-8721.
- Portal help: DHS offices hold Technology Adoption Days on Wednesdays for people who need help using online tools.
Quick-reference table
| If this is your situation | Start here | Why |
|---|---|---|
| You have low income and Medicare bills | QMB | QMB can pay Medicare premiums and protect you from Medicare-covered cost-sharing. |
| Your income is above QMB but still modest | QI | QI can pay the Part B premium, but it does not give QMB billing protection. |
| An old guide says you may be SLMB | Ask about QMB | Rhode Island now moves many former SLMB-level cases into QMB. |
| You are working with a disability and lost free Part A | QDWI and TTW | This is a special path. Rhode Island points working people with disabilities to Sherlock or Ticket to Work. |
| You have a QMB bill in collections | Billing office first | The fastest fix may be corrected billing, not a new application. |

Who should start with QMB or QI
Most older adults in Rhode Island should first look at QMB or QI. These are the two public tracks shown for most applicants on the state’s current Medicare Premium Payment chart.
QMB means Qualified Medicare Beneficiary. It is the stronger path. It can pay Medicare Part A premiums, Part B premiums, and Medicare-covered deductibles, coinsurance, and copayments. It also gives billing protection. Medicare providers should not bill a QMB member for Medicare-covered cost-sharing.
QI means Qualified Individual. It pays the Medicare Part B premium only. It can still be a big help because the Part B premium comes out of many people’s Social Security checks each month. But QI does not protect you from doctor and hospital cost-sharing the way QMB does.
Rhode Island’s 2026 change matters because older pages may still tell readers to look for SLMB as a separate practical path. Federal law still includes SLMB. But Rhode Island says people who would have qualified for SLMB now qualify for QMB instead and are automatically enrolled in the QMB group.
If you want the national background before comparing states, our national MSP guide explains the four MSP names in more detail.
2026 income and resource limits
The current Rhode Island limits are higher than the basic federal floor for QMB and QI. These numbers are monthly income limits. The resource limits are the same for QMB and QI.
| Program | Single monthly income | Married couple monthly income | Resource limit | Main help |
|---|---|---|---|---|
| QMB | $1,683 | $2,275 | $9,950 single; $14,910 couple | Part A, Part B, and Medicare-covered cost-sharing |
| QI | $2,255 | $3,050 | $9,950 single; $14,910 couple | Part B premium only |
| SLMB | No separate public 2026 Rhode Island chart for most applicants | No separate public 2026 Rhode Island chart for most applicants | Handled through the state’s current MSP rules | Former SLMB-level cases are now placed in QMB |
| QDWI | Federal 2026 limit: $5,405 | Federal 2026 limit: $7,299 | $4,000 single; $6,000 couple | Part A premium only |
Example: A single senior in Providence with $1,600 in countable monthly income may fit QMB if resources are also under the limit.
Example: A married couple in Middletown with $3,000 in countable monthly income may be above QMB but inside QI if resources are under $14,910.
Do not use an old chart if it shows only the basic federal QMB and QI amounts for Rhode Island. The current state chart is the safer source. For federal context, Medicare.gov also explains that some states count income and resources differently.
How QMB, QI, SLMB, and QDWI work
QMB in Rhode Island
What it helps with: QMB is the best fit when the main problem is Medicare premiums plus doctor or hospital cost-sharing. It can help with Part A premiums if owed, Part B premiums, deductibles, coinsurance, and copayments for Medicare-covered care.
Who may qualify: Medicare beneficiaries age 65 or older and adults with disabilities may qualify if they meet Rhode Island’s income and resource limits.
Where to apply: Use HealthyRhode RI, the paper MPP-1 form, DHS by phone, mail, or a DHS office.
Reality check: QMB billing protection is for Medicare-covered services and items. It does not make every non-Medicare service free.
QI in Rhode Island
What it helps with: QI pays the Medicare Part B premium. It does not pay deductibles, copays, or coinsurance.
Who may qualify: People with Medicare may qualify if they are within the QI income and resource limits and do not already qualify for other Medicaid coverage.
Where to apply: Use the same Rhode Island application routes as QMB.
Reality check: QI funds are limited. Medicare says states approve QI on a first-come, first-served basis, and people who had QI last year get priority if they still qualify. QI must be applied for each year.
SLMB after Rhode Island’s 2026 change
What it helps with: SLMB is a federal MSP category that usually pays the Part B premium. In Rhode Island’s current setup, the practical result is different. People who would have qualified for SLMB are now placed in QMB.
Who may qualify: If an old article, old notice, or out-of-state helper says “SLMB,” ask DHS how your case is handled under Rhode Island’s current QMB rule.
Reality check: Older national guides can be wrong for Rhode Island in 2026. Keep a copy of the state page or your notice if a provider or helper questions your status.
QDWI and working-disabled help
What it helps with: QDWI pays the Part A premium only for a narrow group of working people with disabilities who lost premium-free Part A after returning to work.
Who may qualify: Medicare.gov says QDWI is for people who have a disability, are working, and lost Social Security disability benefits and premium-free Part A because they returned to work.
Where to apply: Rhode Island points working people with disabilities to Sherlock and TTW. The Office of Rehabilitation Services is listed at 1-401-421-7005, TTY 1-800-745-5555, and Spanish 1-401-272-8090 on the DHS office page.
Reality check: QDWI is not the usual path for most retired seniors. It mainly matters if work, disability status, and Part A premium loss are all part of the problem.

How to start without wasting time
Start with the route you can finish. A perfect online application that never gets submitted will not help. A complete paper form with proof can be better than a half-finished portal account.
- Pick your path. Apply online, use the paper MPP-1 form, call DHS, mail the form, or use a DHS office drop box.
- Use the right form. If you only need Medicare cost help, the MPP-1 form is the focused choice. If you want Medicaid, SNAP, or other help too, the DHS application page explains broader application routes.
- Do not leave resource questions blank. Rhode Island still checks resources for QMB and QI.
- Keep proof of filing. Save the upload receipt, take a photo of the drop box envelope, or keep a full copy of the mailed packet.
- Watch for document requests. DHS may send an Additional Documentation Required notice. The acceptable documents page explains that another document may sometimes be used if you do not have the exact proof listed.
If the portal is the main problem, our HealthyRhode portal guide may help you decide when to use online tools and when to call or go in person.
Documents to gather
Gather documents before you apply if you can. Missing proof is one of the most common reasons a simple case becomes a delayed case.
| Document or information | Why it matters |
|---|---|
| Medicare card or Medicare number | DHS needs to confirm the Medicare coverage involved. |
| Social Security award letter | This helps show monthly income. |
| Pension or work income proof | Other income may count. |
| Recent checking and savings balances | Rhode Island has resource limits. |
| IRA, annuity, burial, or life insurance records | These may affect the resource review. |
| Other health insurance cards | DHS may need to know about other coverage. |
| Spouse income and assets | Married-couple rules can matter even if only one spouse has Medicare. |
| Denial, closure, or medical bill | This helps if you are trying to fix a current problem. |
If you are close to the resource limit: Do not give money away or retitle property just to try to qualify. Rhode Island’s resource reduction notice describes allowed ways certain expenses may reduce countable resources. Ask DHS or a benefits counselor before moving assets.
How long approval can take
Rhode Island Medicaid rules say Community Medicaid for elders age 65 and older uses a 30-day determination timeline after a complete application is received. Adults with disabilities may have a 90-day timeline when a disability determination is needed. Those timelines come from the state’s IHCC rules.
Real life can take longer if the application is not complete. A case can stall if DHS needs bank records, spouse information, insurance details, signatures, or an asset verification consent.
After approval: Keep the approval notice. If you have QMB, show both Medicare proof and Medicaid or QMB proof at appointments. If you have Original Medicare, your Medicare Summary Notice may also show QMB status.
Renewal matters: Do not ignore renewal mail. QI needs special attention because Medicare says it must be applied for each year.
What to do about a QMB bill
A QMB member should not be billed for Medicare-covered deductibles, coinsurance, or copayments. Billing mistakes still happen. Use this path before you pay.
- Check the service date. Make sure you had QMB on the date of care.
- Call the billing office. Tell them you are QMB and ask them to rebill.
- Send proof. Give your Medicare number and Medicaid or QMB proof.
- Ask for a hold. If the bill is near collections, ask the provider to pause collection activity while the QMB issue is checked.
- Escalate if needed. Ask the provider to use Rhode Island’s claims processing page. The provider help desk numbers are 1-401-784-8100 and 1-800-964-6211.
Phone script for a provider bill: “I am a Qualified Medicare Beneficiary. I should not be billed for Medicare-covered cost-sharing. Please check my QMB status for the date of service and rebill this correctly.”

Phone scripts that may help
| Who you are calling | What to say |
|---|---|
| DHS about an application | “I applied for the Medicare Premium Payment Program. Can you tell me if any proof is missing and whether an ADR notice is waiting in my account?” |
| DHS about old SLMB wording | “My notice or old paperwork mentions SLMB. How is my case handled under the 2026 QMB rule?” |
| HealthSource RI about appeal filing | “I received a Medicaid or MSP decision I disagree with. I need help filing an appeal and asking about aid pending.” |
| Drug plan after approval | “Rhode Island approved my Medicare Savings Program. Can you check whether my Extra Help record has updated?” |
Denied, delayed, or blocked
Do not stop at “denied” if the notice is unclear. First find out the exact reason.
- If proof is missing: Submit it quickly through the portal, by drop box, or at a document scanning center. Keep copies.
- If resources are the issue: Ask whether allowed resource reduction rules could help before you move money.
- If income is too high: Ask whether you should be screened for another Medicaid path.
- If you disagree: Use the DHS appeal page. You can appeal through HealthyRhode RI, call HealthSource RI at 1-855-840-4774 for help, fax 1-401-223-6317, or go to a DHS office.
- If benefits are stopping: The appeal rights chart says asking within 10 days can help with aid pending in many cases. Read your own notice carefully.
- If you need legal help: Rhode Island Legal Services may be able to help low-income people with benefit problems. Call 1-401-274-2652 or 1-800-662-5034.

Reality checks and common mistakes
- Old Rhode Island MSP charts are risky. Many still show the pre-2026 SLMB setup.
- QI is not the same as QMB. QI pays Part B only. QMB has stronger billing protection.
- Missing spouse details can slow a married case. Do not assume the other spouse is ignored because only one spouse has Medicare.
- Blank asset sections cause trouble. List accounts and policies honestly.
- Do not pay a QMB bill too fast. Check the date of service and ask for rebilling first.
- Portal problems are not a reason to wait for weeks. Use the phone, a drop box, a DHS office, or a Wednesday Technology Adoption Day.
- Do not count on automatic renewal without checking mail. QI needs annual action.
Backup options
If MSP does not solve the whole problem, look at nearby help. Do not use these options as a replacement for applying for MSP if you may qualify. Use them as backup paths.
- Prescription help: QMB and QI should also connect to Extra Help for drug costs. Our Extra Help guide explains what to check if drug costs do not drop.
- Full Medicaid: Some people need more than premium help. Ask DHS to screen all Medicaid pathways that fit your age, disability, income, and medical need.
- Housing pressure: Medicare help may free up cash, but it will not solve rent by itself. See Rhode Island housing help if housing is the bigger emergency.
- Local aging support: Aging network workers may help you sort mail, forms, transportation, meals, and other needs. Start with Rhode Island aging agencies.
Local help in Rhode Island
Rhode Island uses one statewide MSP rule set. Local differences are mostly about access: phone help, document drop boxes, office visits, and counseling.
| Help source | Contact | Best use |
|---|---|---|
| DHS call center | 1-855-697-4347; relay 7-1-1 | Apply, ask about missing proof, check a notice, or ask where to send documents. |
| DHS offices | DHS office list | Drop off forms, use document scanning, or get Technology Adoption Day help. |
| Office of Healthy Aging | 1-888-884-8721 | Free Medicare counseling and help understanding options. |
| ADRC | 401-462-4444 | Connection to aging and disability resources. |
| HealthSource RI | 1-855-840-4774 | Appeal filing help and health coverage questions. |
DHS lists Providence, Pawtucket, Woonsocket, South County, and Newport County as walk-in offices. It also lists the Shepard Building and Hazard Building as appointment-only locations. Walk-in regional office service is generally Monday, Tuesday, Thursday, and Friday from 8:30 a.m. to 4 p.m., except holidays. Technology Adoption Days are held on Wednesdays.
Rhode Island also posts the MPP-1 in Spanish form and Portuguese form versions. Ask for interpreter help if you need it.
Frequently asked questions
Does Rhode Island still have SLMB in 2026?
Federal law still includes SLMB. But Rhode Island says that as of February 1, 2026, people who would have qualified for SLMB will now qualify for QMB instead and be automatically enrolled in the QMB group.
What are the 2026 Rhode Island QMB and QI income limits?
For QMB, the monthly income limit is $1,683 for one person and $2,275 for a married couple. For QI, it is $2,255 for one person and $3,050 for a married couple.
What are the 2026 resource limits?
The resource limit for QMB and QI is $9,950 for one person and $14,910 for a married couple. Rhode Island may ask about accounts, retirement funds, annuities, life insurance, property, and transfers.
Does QMB mean full Medicaid?
Not always. QMB-only is Medicare cost help. It is not the same as full Medicaid. Ask DHS to screen you for all Medicaid paths if you need broader health coverage or long-term care help.
Will MSP approval give me Extra Help?
QMB and QI should also connect you to Extra Help with Medicare drug costs. If your drug costs do not change after approval, call your drug plan and ask whether your Extra Help record has updated.
What should I do if a doctor bills me and I have QMB?
Call the provider before paying. Say you are a Qualified Medicare Beneficiary and ask the office to rebill the charge correctly. If that does not work, call Medicare, DHS, or a SHIP counselor.
How do married-couple rules work?
Rhode Island posts married-couple income and resource limits. The state may review spouse income, spouse assets, insurance, and living arrangements even if only one spouse has Medicare.
What if my application is denied or delayed?
Ask for the exact reason. If proof is missing, send it quickly and keep copies. If you disagree with the decision, file an appeal by the deadline on your notice and ask about aid pending if your help is stopping.
