If You Have a Denial or Billing Deadline
Do not wait for a later review of this guide. Read the date on your notice. Louisiana Medicaid eligibility decisions can be appealed, and the deadline is tied to the notice. If a provider is billing you even though you have QMB, contact the provider now and keep the bill, date of service, and your QMB proof.
What Has Changed
Louisiana’s 2026 income chart took effect March 1, 2026. This update also makes the difference between countable income and gross income clearer. The state’s QMB threshold is $1,330 for one person and $1,804 for an eligible couple after applicable income disregards; SLMB and QI use higher poverty-level bands.
There is also an important date ahead. Louisiana says its general Medicaid retroactive-coverage rules change on October 1, 2026. Adults age 65 or older and people with disabilities will generally have up to two prior months of possible retroactive Medicaid eligibility instead of three. QMB itself is not retroactive. Older Louisiana MSP manual language still describes up to three prior months for SLMB or QI, so applicants filing on or after October 1 should ask Medicaid how the new rule applies to their MSP category. What Each Louisiana MSP Pays
Medicare Savings Programs are Medicaid programs that help with Medicare costs. They are not cash grants. The national Medicare MSP overview explains the four groups, while Louisiana Medicaid makes the final decision for Louisiana residents.
QMB: the strongest protection
Qualified Medicare Beneficiary (QMB) can pay the Part B premium, the Part A premium if you owe one, and Medicare deductibles, coinsurance, and copayments for Medicare-covered services. If you have QMB, federal law protects you from being billed for Medicare cost sharing.

SLMB: Part B premium help
Specified Low-Income Medicare Beneficiary (SLMB) pays the Medicare Part B premium. It does not give the same Medicare cost-sharing protection as QMB. It may still be valuable because the monthly Part B premium can otherwise come out of a Social Security payment.
QI: Part B premium help
Qualifying Individual (QI) also pays the Part B premium. QI is limited by annual federal funding. Medicare says states approve QI on a first-come, first-served basis and give priority to people who received QI the previous year. QI must be renewed each year.
QDWI: a narrower working-disabled program
Qualified Disabled and Working Individual (QDWI) is mainly for some people under age 65 who have a disability, returned to work, and lost premium-free Part A because of earnings. It helps with the Part A premium. Most retired seniors will not use this category.
2026 Louisiana Income and Resource Rules
Louisiana’s 2026 poverty chart uses the following countable-income bands beginning March 1, 2026. These numbers are not always the same as your gross Social Security check, pension, or wages.

These are Louisiana countable-income standards, not a do-it-yourself gross-income test. Louisiana’s public 2026 Medicaid limits also lists $1,330/$1,804 for MSP help that includes Medicare cost sharing and $1,796/$2,435 as the upper limit for Part B-premium-only help.
For QMB, SLMB, and QI, the national Medicare screen shows limits that are $20 higher because the standard general income disregard is built into those public screening figures. Louisiana’s table above shows the countable-income poverty levels used after the state applies income rules. If your income is close, apply rather than trying to settle eligibility from either chart alone.
Why Your Gross Income Can Be Higher
Louisiana Medicaid uses SSI-related income rules for these programs. For example, the state can subtract a general income disregard before deciding how much income counts. Earned income can receive additional deductions. That is why a person whose gross income is above a table number may still qualify.
This matters especially for QDWI. Do not compare a worker’s full paycheck directly with the $2,660 or $3,607 countable-income figure. Let Medicaid calculate the work-income deductions.

Resources are different in Louisiana
Louisiana currently disregards resources for QMB, SLMB, and QI. Savings in a bank account do not create a separate asset limit for those three groups. QDWI is different: Louisiana uses a $4,000 resource limit for one person and $6,000 for a couple.

How to Apply Without Wasting Time
Louisiana offers four practical routes: online, phone, paper, or in-person help. The state says Medicaid Customer Service is available Monday through Friday from 8 a.m. to 4:30 p.m. at 1-888-342-6207. TTY users can call 1-800-220-5404.
- Choose one main route. Use the online portal, call Medicaid, or complete the paper application.
- Ask for all MSP screening. Do not ask only for QMB if your income may fit SLMB or QI.
- Give spouse information. Louisiana may use couple budgeting when spouses live together under MSP rules.
- Answer every question. Even when resources do not count for QMB, SLMB, or QI, incomplete answers can delay a case.
- Keep proof. Save the date, confirmation number, copies, notices, and names from phone calls.

How long can a decision take?

QMB Billing Protection: Do Not Pay the Wrong Bill
If you have QMB, Medicare providers and suppliers generally cannot bill you for Medicare Part A or Part B deductibles, coinsurance, or copayments for Medicare-covered services. This rule applies even if the provider says it does not accept Medicaid. Show your Medicare card and proof of QMB status. Ask the billing office to correct the claim and stop collection activity. If you already paid an improper Medicare cost-sharing bill, ask the provider to review the payment and refund any amount you did not owe.

Denied, Delayed, or Still Paying the Premium
If you are denied
Read the notice before doing anything else. Check which MSP group was reviewed, the household size, the income month, and whether income disregards were used. If the case is delayed
Call Medicaid and ask exactly what is pending: income proof, spouse information, Medicare verification, or another eligibility item. If your application is close to the 45-day limit, ask whether the case is complete and whether any proof request was mailed or posted online.
If Part B is still withheld
Keep your approval notice. Ask Medicaid whether the Medicare buy-in information was sent. If the deduction continues, call Social Security or Medicare and ask when the change should appear. Do not throw away notices showing the covered month.
