For many older adults in Alabama, a Medicare Savings Program means the state starts paying the standard Medicare Part B premium. The current CMS premium notice lists the 2026 standard Part B premium as $202.90 a month. If you qualify for QMB, the help can be bigger because QMB also protects you from many Medicare deductibles, coinsurance, and copayments for Medicare-covered care.
The most important Alabama rule is simple: Alabama’s 2026 income limits say resource limits do not apply to the Medicare Savings Program. Important number note: Alabama’s Form 210 Medicare Savings Program handout still shows $1,824 as the QI individual upper limit and also shows older projected Medicare premium and deductible figures. The newer Alabama income limits handout and the current federal Medicare page show the QI individual upper limit as $1,816. This guide uses $1,816 as the safer number. If your income is close to the limit, apply anyway and let Alabama Medicaid make the decision.
Who qualifies
In Alabama, the basic rules are the same starting point for most MSP applicants. You must live in Alabama, be eligible for Medicare Part A, and be a U.S. citizen or have satisfactory immigration status. You must also have income under the correct program limit.
For married couples, Alabama uses an important rule. If both spouses have Medicare, their combined income must stay under the couple limit. If only one spouse has Medicare, the spouse applying must stay under the individual limit, and both spouses together must stay under the couple limit.
QDWI is different. Under Alabama’s eligibility code and current federal rules, QDWI is only for someone under age 65 who is disabled, working, lost premium-free Part A after returning to work, and is not otherwise eligible for Medicaid.

Qualified Medicare Beneficiary (QMB)
What it is: QMB is the strongest Medicare Savings Program in Alabama. It is the one to check first if your income is low and you need protection from Medicare bills.
Who can get it: Someone who meets Alabama’s basic MSP rules and stays at or below the QMB income limit of $1,350 a month for one person or $1,824 for a couple.
How it helps: QMB pays the Part B premium. It may also pay the Part A premium when needed. QMB also protects you from deductibles, coinsurance, and copayments for Medicare-covered care.
How to apply or use it: Use Form 211. After approval, show both your Medicare and Medicaid cards each time you get care.
What to know first: Alabama’s handout still tells people to use providers who accept Medicaid. That is practical advice because it can make billing smoother. But federal QMB billing rules say Medicare providers and suppliers cannot bill QMB members for Medicare Part A or Part B cost-sharing, even if the provider does not accept Medicaid. Specified Low-Income Medicare Beneficiary (SLMB)
What it is: SLMB is the next step up if your income is a little too high for QMB.
